Hours | 35 hours per week

Working in primary care settings across Birmingham, Primary Care Navigators will work to support patients who are experiencing poor mental health and wellbeing. You will work collaboratively with clinical staff and patients to support their recovery; action planning and signposting to relevant agencies.
